Submit Forms & Reports

Submit one of the following forms to the chapter:

• Committee report – used to present committee status to the Chapter during regular Chapter meetings.
• Nomination form (generally between February and March) – used to submit a nomination for an officer or director position with the chapter.
• Calendar event – used to both create a calendar event and submit an insurance form for formal Chapter sponsored events involving any brothers from the EAC.
